Once Upon A Time Early Learning Center currently has a waiting list for new students.
Families may apply for an enrollment spot by completing the Child Care Waiting List Form.
The Enrollment Process
APPLICATION
If you wish to enroll your child in our program, you may download a Child Care Enrollment application or you can call the center to request one. Please return the completed application form to the center. There is no application fee.

Requests for enrollment are processed on an age-eligibility and date-of-application basis with the following priorities:

Families already enrolled at the center (siblings)

Children of members of staff
-
All other families
We may not have an enrollment opening at the exact time that your family needs enrollment to begin.

It is the responsibility of the prospective family to assure that all application information is kept current.
​
When we can offer you an appropriate space for your child, we will contact you. Please feel free to call 724.222.6180 with any questions you may have regarding our center.

ENROLLMENT PROCEDURE
​
We encourage families to take the time to visit, observe and ask questions before making a decision on enrollment. When you decide to enroll your child in our center, you may wish to transition your child into the center with some short visits.
​
Please inform us if you require assistance translating forms, agreements, policies or procedures.
​
The following forms are required to complete your child’s file. As we are required to maintain this information on file by state law, and periodically have you review it, failure to provide this information, including timely updates, may result in termination of care.

Children’s records must contain the following information:

Child Health Report - this form must be signed by your child's doctor and returned to us within 60 days of enrollment. There are no exceptions - it is required by state law. The family is responsible for keeping the child’s immunizations and screenings up to date. Child Health Reports are required periodically during your child's enrollment, at regular intervals, as determined by state law.

Emergency Contact and Authorization to Release forms- required before your child begins at our center. Families must supply us with information regarding emergency contacts for your child. Parents/Legal Guardians are the first contact in the event of any emergency, but you may provide additional contact persons with phone numbers and addresses, in the event that you cannot be reached.

Fee Agreement [This will be provided to you at the time of your application request]

Food Program (CACFP) Form – In order for our center to participate in the CACFP (Child and Adult Care Food Program) we must have completed forms from all families.

We additionally request that you provide the following information:

Consent Form (Saline Nose Wipes, Sunscreen, Hand Sanitizer, Toothpaste, Lip Balm)
